Your assigned operations officer has contacted you by e-mail or in person to discuss the timeline for the upcoming annual self-evaluation process. Please be sure to return the form to them promptly based on the provided timeline. Note: This is a required process for membership with CHS. Duty Uniforms – Change Effective 1/1/2018 As of January 1, 2018 all job shirts and sweaters must be logoed with CHS MIHC logo. HVA, CVAS, and Scottsville job shirts should be retired. Old job shirts are yours and can be worn while off duty, however they will not be allowed on duty. Please remember that although our corporate name has changed, HVA, CVAS, SRS are still well known to the community. Ambulance Seals
Ambulance Seals Effective immediately, there is no longer any need to date the seals on the ambulance cabinets which contain items that expire. Now that we will be inspecting this equipment on the 15th of each month, expired items will be replaced at that time should they expire before the 15th of the following month.

Jesse Merdak Promoted to Lieutenant
Please join me in congratulating Jesse Merdak on his promotion to Lieutenant effective on Monday September 25th. Jesse will be going through a training program over the coming weeks. There will be a formal swearing in at our next staff meeting in October.
Vehicle Inspections in ePro
As mentioned at the training meeting on Tuesday, the ambulance inspection forms in ePro have been updated and will be available for use starting Monday 9/11/17. A reminder for double BLS ambulance crews: both the ALS and BLS checks must be completed. For confirmation that the CS kit seal matches the CS book, EMTs may select “not applicable” on the form. Long Sleeve Uniform Shirts
It’s time to order long sleeve shirts. Please place your order as soon as possible using the Uniform Request form. Lt Neufeglise needs to have all requests by Friday, August 11th. FT: 5 shirts; PD: 2-4 depending on hours, if you’re not sure talk to me. Volunteers: 1-2 shirts. Quantities are based on hours.
